On 10/25/2015, the vessel LEVIATHAN II (Official Number 800190) was involved in a person seriously injured or killed - boarding, being on board, falling overboard from the ship in Plover Reefs, Clayoquot Sound, BC, BRITISH COLUMBIA (BC).
The vessel, with 3 persons on board, experienced the incident while engaged in regular operations. Fortunately, the ship was able to continue its voyage. No injuries or fatalities were reported, and there was major pollution.

On 25 October 2015, the passenger vessel "LEVIATHAN II", with 27 people on board, reported as capsized off Plover Reefs in Clayoquot Sound, BC while on a whale watching excursion. The subsequent search and rescue operation recovered 18 passengers and 3 crew members, all of whom were transferred to Emergency Services at Tofino, BC. There were five fatalities and one missing person. Approximately 2000 litres of fuel leaked into the sea.
