On 8/12/2009 , the vessel FEN (Official Number 393484) was involved in a person seriously injured or killed - in contact with any part of the ship or its contents in LOCK 7, WELLAND CANAL, ON, ONTARIO (ON).

The vessel, with 19 persons on board, experienced the incident while engaged in regular operations. Fortunately, the ship was able to continue its voyage. No injuries or fatalities were reported, and there was pollution.

On 12 August 2009, while the "FEN" was secured in Lock 7 Welland Canal and as the lock was filling, the ship surged on its mooring snapping a steel wire against the 3rd mate's left foot. The 3rd mate was taken to a hospital in St. Catharines, Ontario
