On 2/9/1999 1, the vessel NORTH ISLAND STAR (Official Number 391720) was involved in a total failure of any machinery or technical system in SW QUEEN CHARLOTTE ISLANDS, B.C., BRITISH COLUMBIA (BC).

The vessel, with 3 persons on board, experienced the incident while engaged in regular operations. Fortunately, the ship was able to continue its voyage. No injuries or fatalities were reported, and there was pollution.
