On 1/18/1989 , the vessel MIZUKAWA MARU (Official Number 125222) was involved in a person seriously injured or killed - in contact with any part of the ship or its contents in OFF TOFINO, VANCOUVER ISLAND, B.C.,.

The vessel, with 19 persons on board, experienced the incident while engaged in regular operations. Fortunately, the ship was able to continue its voyage. No injuries or fatalities were reported, and there was pollution.
