On 9/11/1992 , the vessel COSMOS VENTURE (Official Number 8667) was involved in a person seriously injured or killed - in contact with any part of the ship or its contents in 49 NM OFF CLAYOQUOT SOUND, B.C.,.

The vessel, with 22 persons on board, experienced the incident while engaged in regular operations. Fortunately, the ship was able to continue its voyage. No injuries or fatalities were reported, and there was pollution.

This incident was officially recorded by Transport Canada on 5/26/1994 . Weather at the time was reported as snow with winds from the NW at 6.00 knots.
