On 15 September 2019, the tug "SEASPAN CAVALIER", with the loaded barge "HANALEI" in tow and another tug to assist, reported a deckhand from the assist tug boarded the barge to check stern gap and fell overboard from the barge while the tug and barge were approaching alongside in Vancouver Harbour, BC. The deckhand was wearing a Personal Floatation Device (PFD). He swam and boarded a nearby vessel. No injury was reported.
