On 2/21/1997 , the vessel OSPREY ARROW (Official Number 708333) was involved in a person seriously injured or killed - in contact with any part of the ship or its contents in KITIMAT, B.C.,.

The vessel, with 21 persons on board, experienced the incident while engaged in regular operations. There were 2 injuries but no fatalities were reported, and there was pollution.

Bosun & one O/S were trapped and squeezed by the moving ship's gantry while working aboard the "OSPREY ARROW". Bosun sustained serious injury, was hospitalized and underwent surgery. The O/S was released from the hospital after initial checkup.. Damage to the vessel was indicated as
