The vessel, with 2 persons on board, experienced the incident while engaged in regular operations. There were No injuries but 2 fatalities were reported, and there was unknown pollution.

F/V "LADY KAY No. 1" found capsized and aground on a reef off Bella Bella on 21 December 1996. Both crewmen died in the accident. This was reported to TSB on 29 June 1998.. Damage to the vessel was indicated as CONSTRUCTIVE TOTAL LOSS
